Rádiologická diagnostika ako podklad atypického interceptívneho rozhodnutia: kazuistika extrakcie zárodku laterálneho rezáka u nespolupracujúceho detského pacienta

Predkladaná kazuistika opisuje neštandardné interceptívne rozhodnutie u 8-ročného detského pacienta indikovaného na ošetrenie chrupu v celkovej anestézii pre výrazne obmedzenú spoluprácu. Pri vstupnom vyšetrení bola na panoramatickej snímke zistená prítomnosť nadpočetného zuba vo frontálnom úseku hornej čeľuste. Doplňujúca zobrazovacia diagnostika pomocou CBCT preukázala mesiodens v polohe zodpovedajúcej oblasti zuba 21, zatiaľ čo stály zub 21 sa vyvíjal distálnejšie, v oblasti laterálneho rezáka. Po medziodborovej úvahe a po informovaní rodičov bol zvolený atypický postup: ponechanie mesiodensu v pozícii budúceho centrálneho rezáka a extrakcia zárodku zuba 22. Cieľom bolo využiť priaznivú morfológiu a polohu mesiodensu, akceptovať zub 21 v pozícii laterálneho rezáka a znížiť potrebu rozsiahlejšej budúcej ortodontickej intervencie. Prípad poukazuje na význam individuálneho plánovania, optimalizovanej rádiologickej diagnostiky, medziodborovej spolupráce a dôslednej dokumentácie pri rozhodovaní u nespolupracujúcich detských pacientov.

Radiological Diagnosis as the Basis for an Atypical Interceptive Decision: A Case Report of Germectomy of a Lateral Incisor in an Uncooperative Paediatric Patient

The presented case report describes an unconventional interceptive treatment decision in an 8-year-old paediatric patient indicated for comprehensive dental treatment under general anaesthesia due to markedly limited cooperation. During the initial examination, panoramic radiography revealed the presence of a supernumerary tooth in the anterior maxillary region. Supplementary CBCT imaging demonstrated a mesiodens in a position corresponding to the region of tooth 21, while the permanent tooth 21 was developing more distally, in the region of the lateral incisor. Following interdisciplinary consideration and after informing the parents, an atypical treatment approach was selected: retention of the mesiodens in the position of the future central incisor and extraction of the tooth germ of tooth 22. The aim was to make use of the favourable morphology and position of the mesiodens, to accept tooth 21 in the position of the lateral incisor, and to reduce the need for more extensive future orthodontic intervention. The case highlights the importance of individualised treatment planning, optimised radiological diagnostics, interdisciplinary cooperation, and meticulous documentation when making treatment decisions in uncooperative paediatric patients.
